▪ Registration and Fee
- All fees are non-refundable.
- Products and Accessories purchased cannot be exchanged or refunded.
- Utilization of all packages should start within one month of the purchase date. Package will
lose validity after 30 days of purchase. There will be no refund in case of expiry of the
stipulated period.
- Packages purchased are not transferrable to any other person whether related or unrelated.
- Gymnastics membership cannot be frozen for a future date. It must be utilized in the same
term.

▪ Schedule and Make Up Classes
- Class timing are subject to availability and schedule set by management.
- Make up classes if any should be utilized in the same term and will not be carried forward to
the next term.
- Maximum of 3 make-up classes per term are allowed and medical report should be provided
for the same to be considered.
- Make-up class should be booked a week in advance so that we can schedule them in a timely
manner.
- Kids without booking will not be accepted for the class.
- While we will do our best but it is possible that the for make-up classes the coach can be
different from the regular coach.
- Lvl Up Gyms will reserve the same slots for renewal of the registration from one term to
another for only 1 week after opening registration.

▪ Code of Conducts
- Students must arrive 5 minutes before classes
- Late Student: The Coaches will not extend the class beyond time. (regular class and Private
training), as it disturbs the schedule for other students.
- Private sessions must have cancelled or rescheduled with at least a 12-hour notice. In case no notice is received the class will be charged for.
- Parents are requested to not come inside the gym, except mommy and me classes. It hampers with the coaching and also the concentration of the child.
- Level Up reserve the right to change the coach at any necessary time, the student will be assigned to a new coach in case of necessity. This will only be done in case in if it unavoidable.
When discrepancy occurs between the understanding of the information provided by level up employees, the written terms and conditions will prevail.

Waiver and Release of Liability:
Express assumption of risk: I, the undersigned, am aware that there are significant risks involved in all aspects of physical training. These risks include but are not limited to falls which can result in serious injury or death; injury or death due to negligence on the part of myself, my training partner, or other people around me; injury or death due to improper use or failure of equipment; strains and sprains. I am aware that any of these above-mentioned risks
may result in serious injury or death to myself and/or my partner(s). I willingly assume full responsibility for the risks that I am exposing myself to and accept full responsibility for any injury or death that may result from participation in any activity or class while at or under direction of LVLUP GYMS.

I acknowledge that I have no physical impairments, injuries or illnesses that will endanger myself or others.

Release: In consideration of the above mentioned risks and hazards and in consideration of the fact that I am willingly and voluntarily participating in the activities offered by LVLUP GYMS, I, the undersigned hereby release LVLUP GYMS, their principals, agents, shareholders, representatives, employees, and volunteers from any and all liability, claims, demands, actions or rights of action, which are related to, arise out of, or are in any way connected
with my participation in this activity, including those allegedly attributed to the negligent acts or omissions of the above mentioned parties. This agreement shall be binding upon me, my successors, representatives, heirs, executors, assigns, or transferees. If any portion of this agreement is held invalid, I agree that the remainder of the agreement shall remain in full legal force and effect. If l am signing on behalf of a minor child, I also give full permission for any person connected with LVLUP to administer first aid deemed necessary, and in case of serious illness or injury, I give permission to call for medical and/or surgical care for the child and to transport the child to a medical facility deemed necessary for the well-being of the child.

Indemnification: The participant recognizes that there is risk involved in the activities offered by LVLUP GYMS.
Therefore, the participant accepts financial responsibility for any injury that the participant may cause either to him/herself or to any other participant due to his/her negligence. Should the above-mentioned parties, or anyone acting on their behalf, be required to incur attorney’s fees and costs to enforce this agreement, I agree to reimburse them for such fees and costs. I further agree to indemnify and hold harmless LVLUP GYMS s, their principals, agents, shareholders, representatives, employees, and volunteers from liability for injury or death of any person(s) and damage to and/or loss of property that may result from my negligent or intentional act or omission while participating in activities offered by LVLUP GYMS at the main building or abroad. This includes but is not limited to parks, recreational areas, playgrounds, areas adjacent to the main building, and/or any area selected for training by LVLUP GYMS.

Photography/Video Release
Participants involved in any activities offered by LVLUP GYMS may be photographed or videotaped during training. The undersigned hereby consents to the use of these photographs and/or videos without compensation, on the LVLUP GYMS website, social media sites or in any editorial, promotional, or advertising material produced and/or published by LVLUP GYMS.

I have read and understood the foregoing assumption of risk, and release of liability and I understand that by registering, it obligates me to indemnify the parties named for any liability for Injury or death of any person
and damage to and/or loss of property caused by my negligent or intentional act or omission. I understand that
by registering at Level Up Fitness Center this form I am waiving valuable legal rights.
